The online form does NOT resemble the printed form. To use this form, simply click on the icon and complete the form that appears in your window. When you have completed the form, click the SUBMIT button and the form will automatically be emailed to the appropriate receipient(s). You must provide accurate email addresses or the form will not reach its destination. A copy of the form will also be emailed to you providing your email address was supplied.
This form should be completed by the presidential candidate and returned to the Chairman of the Elections Committee (aka Chairman of the Board) no sooner than December 1. With this form the candidate should include two additional documents: (1) A letter from the president of their local chapter declaring the chapter support of their candidacy and explaining how the chapter can support themes president; and (2) A statement of their belief in this organization and why they wish to serve in this office. (Information from these two documents will be used for website and NEWSLET purposes.) These documents should reach the Chairman of the Elections Committee as soon as possible (and no later than 12:00pm Friday at Annual Convention). In addition, please submit an electronic photo of the candidate suitable for media use. No one is officially a candidate until this form is approved by the Elections Committee
